Medical Form 

Along with the safety and security of our campers, we take the health of our campers very seriously. It is vital for our staff leadership to know any allergies or medication requirements that the campers may have.

Dietary Needs:

If your child has special dietary needs, please make sure you call / email our Kitchen Supervisor to make sure we can accommodate your child’s needs for his week of camp. Please make sure you clearly mark on the registration if your child has allergies to foods and what they are. Please remember that if you are bringing special dietary foods to accommodate your child you need to turn them into the kitchen staff with their name and directions on them (this is after you have spoken directly to the Kitchen Supervisor). Foods are not allowed to be kept in the dorms.

Campers are not allowed to have any form of medication in the dorms. Upon arrival, camper medication must be labeled and and dropped off with the camp medical staff.

A medical form should be filled out prior to arrival at the camp. Without a medical form signed by the parents or guardian, campers will not be allowed to stay at the camp.

You may fill and submit a medical form electronically by following the link below. Paper copies are also available at check-in. If medications need to be checked in with the camp medical staff, an additional form will be available at check-in.
