Medical Form 

Along with the safety and security of our campers, we take the health of our campers very seriously. It is vital for our staff leadership to know any allergies or medication requirements that the campers may have.

Dietary Needs:

If your child has special dietary needs, please make sure you call / email our Kitchen Supervisor (Michelle Frazier) to make sure we can accommodate your child’s needs for his week of camp. Please make sure you clearly mark on the registration if your child has allergies to foods and what they are. Please remember that if you are bringing special dietary foods to accommodate your child you need to turn them into the kitchen staff with their name and directions on them (this is after you have spoken directly to Michelle Frazier). Foods are not allowed to be kept in the dorms.

Campers are NOT allowed to have any form of medication in the dorms. Upon arrival, camper medication will be labeled and and dropped off with the camp medical personnel.

A medical form will be need to be filled out prior to arrival at the camp. Without a medical form signed off by the parents or guardian, campers will NOT be allowed to stay at the camp.

Parents: please download the medical form and bring it completed to registration. This will save you a lot of time during the registration process. If you are mailing your mailing your registration form, please include this medical form.
